Information required at time of the assessment

The following information is required at time of appointment

  • Photo ID
  • 12 month history & proof of prescription medications
  • Must bring in police report relating to your open case
  • Legal history including all arrest, convictions and/or probation
  • All info regarding metal health diagnosis & treatments
  • All info regarding substance abuse history & treatments
  • All medical diagnosis, treatments and/or surgeries 
  • Family history
  • All treatment completion or educational course certificates



**If your DUI was in one of the out of states listed above, please bring all paperwork and/or packet that was mailed to you by your offending state.


***No refunds! A deposit is required upfront at scheduling, the balance will be due at the time of the appointment. If you fail to show your deposit is forfeited and you must pay again to schedule. 

Additional Information

  • Assessments are scheduled by phone
  • Typically takes an hour
  • Allow a minimum of two weeks for results
  • Bring ALL referral information on where your results will be sent to appointment
